MLS Forms
All NorthstarMLS forms are now available through the WEBForms™ application. If you need a print copy of a form, follow this procedure:
- Click the NorthstarMLS Matrix icon on the left side of this screen and log in to Matrix.
- On the Matrix Home page, click the "WEBForms" link on the blue menu bar just below the tabs. A new window will open with the WEBForms application.
- On the right side of the WEBForms screen, click the "Printable Forms Library" tab.
- Click the "View/Print" link next to the name of the form you want. (For the NorthstarMLS forms, scroll down the list to the ones that start with "NSMLS.")
- Type in an MLS number if you want to auto-fill data from the MLS. Otherwise, leave that blank and click the "View/Print" button.

Completed Change Forms must be faxed to your Association (Not RMLS)
